What to Bring to Your Child’s First Appointment

To ensure a smooth first visit, please bring your child's medical and dental history, insurance information, and any questions you have. Consider bringing a comfort item for your child, like a favorite toy or blanket. If your child takes any medications, please bring a list. Arriving a few minutes early allows time for your child to acclimate to our office, and for you to complete any new patient forms if you haven’t filled them out ahead of time.
